Through otherworldly visions, they discover the horrifying fact that their parents were complicit in selling them to an organ harvesting operation led by a figure known as the Surgeon. But the fragile relationship between Andrew and the demon begins to collapse. Ashley departs to the Human Realm, and Andrew is cast into a liminal space called the In Between. His attempt to regain power by summoning the demon back instead draws the attention of Something Terrifying once more, a being now revealed to be Lord Unknown.
As Lord Unknown forces Andrew to relive key moments from his childhood, the memories expose suppressed emotions, particularly Andrew’s disturbing feelings toward Ashley. Rather than recoil, Lord Unknown offers understanding and suggests they form a partnership. Andrew, broken and desperate, agrees. The episode ends with his acceptance and his step into an unknown dimension alongside this ancient being. Episode 3 does not shy away from psychological extremes or moral collapse—it gives players multiple outcomes, including disturbing non-canonical game overs and an alternate ending where the siblings’ relationship twists into something abusive.
